The BSA incident report form is available for you to use as a vehicle for gathering information if someone is injured at a Scouting activity.  Your unit should provide this report to the Council office as soon as possible after an incident occurs.  The more information you can gather the easier it will be for the correct information to be provided to the National Risk Management Committee.  That group of volunteers is responsible for determining what programs should and should not be part of the Scouting Movements program. 

The Guide to Safe Scouting should always be the first resource on planning your programs and the incident reporting information is in the appendix of this valuable tool.  Remember that if you are at a Council or District sponsored event, there should be someone who is recording this information for you when you make them aware that an incident has occurred.
